This application has to be accompanied by a filled Personal Data Consent from Examination Candidate<a href=\"https:\/\/myexams.gov.mt\/wp-content\/uploads\/2024\/12\/Data-Protection-Notice-JCQ-12-1.docx\"> <\/a><strong><a href=\"https:\/\/myexams.gov.mt\/wp-content\/uploads\/2024\/12\/Data-Protection-Notice-JCQ-12-1.docx\">Form<\/a>.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The form needs to be filled in as follows:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Parts 1 and 3 need to be filled, signed and endorsed by the school. Part 2 needs to be filled, signed and endorsed by a specialist.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">There is also another<strong> Form 9 which&nbsp;must&nbsp;only&nbsp;<\/strong>be used for:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>candidates with communication and interaction needs<\/li>\n\n\n\n<li>candidates with a medical condition (formally diagnosed by a registered specialist)<\/li>\n\n\n\n<li>candidates with sensory and\/or physical needs<\/li>\n\n\n\n<li>candidates with social, emotional and mental healthy needs<\/li>\n\n\n\n<li>candidates with speech, language and communication needs<\/li>\n\n\n\n<li>candidates with learning difficulties who require a computer reader\/reader but <strong>do&nbsp;not&nbsp;<\/strong>require extra time or a scribe<\/li>\n\n\n\n<li><strong>all<\/strong> candidates requiring supervised rest breaks<\/li>\n\n\n\n<li><strong>all<\/strong> candidates requiring extra time only<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Click <strong><a href=\"https:\/\/myexams.gov.mt\/wp-content\/uploads\/2024\/07\/Form-9_FINAL.docx\">here<\/a><\/strong> to fill in Form 9.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The completed form must be sent by email to Ms Raiza Pisani on <a href=\"mailto:raiza.mizzi@gov.mt\">raiza.mizzi@gov.mt <\/a> during normal registrations.
